Primary-Care Coordination

For buildings wanting between-visit coverage

An embedded advanced-practice provider who runs the day-to-day on the days your PCPs aren't in — assessing patients on the units and handing every decision back to your attendings.

  • On-site 5–6 hours a day on the nursing units
  • Clinical phone coverage 8 a.m.–6 p.m. weekdays
  • Joins daily nurse clinical rounds; manages admissions, discharges, sick visits
  • Attends twice-weekly Medicare/UR meetings (Mon/Thu)
